Summary
The agency is a medical model adult day care center supervised by DOH. There is a special program for people having functional limitations. The goals are to continue to maintain patients’ well -being to be longer in the community . Our interdisciplinary team (RN, social workers, nutritionist., CNA, rehabilitation department , therapeutic recreation department ) are helping with daily functions and operations .
